OK Medicaid Reminder-Taxonomy Code/Contract Code

Taxonomy codes are required on all OK Medicaid claims. OHCA uses your NPI, Zip+4 and **Contract Code (if applicable) to find the provider ID and location in their system in order to process claims. Valid Contract Codes for OK Medicaid are listed below:

NI (for Indian Providers billing for non-Indian patients)
DM (for Behavioral Health Facility contracted with DMHSAS)
A (for BH Case Mgmt., Public or Private, Over 21)
C (for BH Case Mgmt., Public or Private, Under 21)
DA (for BH Case Mgmt., DMHSAS Contracted, Over 21)
DC (for BH Case Mgmt., DMHSAS Contracted, Under 21)
G (Group Primary Specialty code 082)
T (Teaching Physician provider type 52)
TS (Teaching Specialist)

If you are unsure of your assigned taxonomy code or contract code you can contact Oklahoma Healthcare Authority at (800) 522-0114, option 5 (Oklahoma City area) (405) 522-6205.

**If you have been assigned a contract code you will be required to place it in Box 19 of
the CMS1500 in the following format:

Example: CN1-G OR CN1 G (Be sure a hyphen or space follows the CN1)

If sending in ANSI 837P format, send in Loop 2300-CN1.

You can submit your taxonomy code to Infinedi using one of the following methods:

• Submit each provider’s individually assigned taxonomy code to us via a link located on your Infinedi Q- Home page.

• Email your taxonomy code to taxonomy@infinedi.net

• You may also submit your taxonomy code in place of your Medicaid Legacy Provider number(s) in Box 24J if sending print image. If you choose this option, you must also send the ZZ qualifier. You may submit your Individual taxonomy on your ANSI 837P claim if filing as a group in Loop 2310B PRV03 or in 2000A PRV02 if filing as an Individual.
