State Farm Now Electronic-Special Requirements

Effective 07/13/09 State Farm claims can be submitted electronically using Payer ID HT005054-001. When submitting Property and Casualty Claims you will be required to send the Property & Casualty Claim Number in Box 11 or Loop 2010BA REF 01- Y4. The claim number will usually be 6 or 9 characters and there should not be anything preceding the claim number (i..e. do not put "CLM" or "CLM# or CLAIM in front of the number). The claim number must be a valid State Farm P&C claim number to be accepted by State Farm. The member ID (Policy Number) should be submitted in Box 1a or Loop 2010BA NM109. In rare cases, if they cannot obtain the policy number from the patient, they may use "unknown" in this field. However, this may delay the processing of the claim. If the claim is not marked as accident; auto, work comp or other then you will be required to supply the Member ID (Policy Number) in Box 1a or Loop 2010BA NM109.
