PPO OKLAHOMA, CCN, HCVM, FIRST HEALTH PPO NAME CHANGE

Effective January 1, 2007 PPO/TPA plans known as CCN, PPO Oklahoma, HCVM and First Health PPO will now be known as First Health Network. These claims will be submitted to one Payer ID of 73159 (which is/was the PPO OK payer id). Payer id 33005 (known as CCN) will eventually be cancelled

Provider Action:

To ensure your claims are routed correctly we ask that you send the Electronic Payer ID in front of the carrier name. (See example listed below)
