OKLAHOMA MEDICAID-REQUIRES TAXONOMY CODE AND NPI ONLY MAY 22, 2008

Infinedi has been notified that OK Medicaid will transition to NPI only for all electronic claims. Beginning May 22, 2008, we will transmit NPI only on all outbound electronic claims. No legacy numbers will be sent.

In addition, Taxonomy codes will be required for all Oklahoma providers sending Medicaid claims. OHCA uses your NPI, Zip+4, taxonomy code and **Contact Code (if applicable) to find the correct provider ID and location code in their system. If you are unsure of your assigned taxonomy code or contract code you can contact Oklahoma Healthcare Authority at (800) 522-0114, option 5 (Oklahoma City area) (405) 522-6205.

Provider Action:

Please submit your taxonomy code to Infinedi LLC using one of the following methods:

• Submit each provider’s individually assigned taxonomy code to us via a link located on your Infinedi Q- Home page
• Email your taxonomy code to taxonomy@infinedi.net

**If you have been assigned a contract code you will be required to place it in Box 19 of
the CMS1500 in the following format:

Example: CN1-XX

You may also submit your taxonomy code in place of your Medicaid Legacy Provider number(s) in Box 24K or 33B. If you choose this option, you must also send the ZZ qualifier.
