REVISED: Oklahoma BC/BS-Onset Date

When submitting claims for OK BCBS the onset date is required.  Please make sure this date appears on your CMS-1500.  The "Date of Current Illness" (also known as the "on-set date") is entered into Box 14 to indicate the date of the first symptom/illness, accident/injury, or last menstrual period (LMP) for pregnancy.

When submitting the UB-04, the "Occurrence Code" and associated date is entered in fields 31-36 and defines a significant event related to the claim. The occurrence code of 11 is used for onset date.
