Not all American Family Insurance claims may be routed electronically. See criteria below.
Claims addressed to Interplan Health Group (Re-Pricer):
If Interplan Health Group network re-pricing plan logo appears on the front side of the patient's insurance card, the claims will be routed to the payer on PAPER. (You may choose to mail your claims from your office or have Infinedi print and mail the claims for you. If you choose the latter option, paper claim charges will be applied to your invoice.)
Claims addressed to American Family Insurance:
If the network re-pricing plan logo for Interplan Health Group does NOT appear on the front of the Patient ID card, Infinedi can route claims electronically via Payer ID AMF11. No special enrollment is required for American Family Insurance.
To ensure the proper routing of your claims to either the payer or the network re-pricer, please address your claims to the entity based on criteria listed above. You may call American Family Insurance at 1-800-333-6886 or access their website www.amfam.com if you have questions about network affiliation or the correct claims address for the re-pricer or payer.
