Provider Credentialing Alert

Please be advised that any changes made to your credentialing status for 2010, must be reported to Infinedi, Insurance Carriers and NPPES as needed.  It is possible that the changes, if not reported properly, could affect your claims payments.

A three-way match of information between your NPPES (NPI Registry), Payer/Medicare credentialing information and provider information on your claim must occur or crosswalk errors may result.  Your batches may get deleted by Medicare or other payers.

Please notify Infinedi if you changed or added any of the following:

·         Group NPI Number

·         Tax ID Number
(Please be aware that the number (SSN or Tax ID) reported in block 25 on CMS-1500 must be the same number on file with Medicare, NPPES and the IRS.)

·         Individual & Group Provider Numbers

·         Changed Business Name or Address

·         Changed from Sole Proprietor to Group

·         Changed from Group to Sole Proprietor

·         Added Provider to Group

·         Purchased a Practice

·         Sold a Practice

Please contact our Customer Service Department with any of the above changes at 1-800-688-8087, or download the Provider Information Sheet from our website and fax it directly to our Client Implementation Department at (918)-249-4460.

Likewise, you will need to make the appropriate changes with your local Medicare contractor, NPPES and update information with your other payers.
